Basic Guidelines to offering Employee Benefits

2-50 Employees-Small Employer

• Must have, at least, 2 Full Time eligible employees
• Employees must be on payroll and eligible for coverage
• Employees must work between 17.5 and 40 Hours (employer sets this hourly requirement)
• Employer must contribute at least 50% of the “employee only” premium
• Employee Waiting Periods set at either 30, 60, or 90 days
• Employer must provide Workers Compensation (not needed for owner-only policy)
• Health Insurance Carriers may require a wage report, Quarterly 132 (other documents may work) at time of enrollment 5 or less employees
• Articles of Incorporation may be needed if the owner is not on payroll
• Oregon Standardized Business Form
• Contract Rates once the policy is sold, are valid for one calendar year. Rates do not change once the policy is in force regardless of fluctuation of employees. You may cancel the policy at any time.
